Insurance coverage for Physiotherapy services at Lost ‘n Found Yoga is based on a pay-and-submit system. Patients pay for each session and receive a receipt that they can submit to their group benefits provider. Receipts can be provided electronically or in paper format. Patients then submit their receipts to their insurance provider for reimbursement directly back to the patient in accordance with their benefits plan.
Patients are encouraged to check with their group benefits administrator and/or their benefits guide regarding elegibility for Physiotherapy coverage and possible limits to coverage including per-session limits, annual limits, per-sesson copayments, etc.
Your appointments and well-being are very important to us. We understand that sometimes, unexpected delays can occur, requiring schedule adjustments. If you need to cancel your appointment, we respectfully request a minimum of 24 hours notice.
Any cancellation or reschedule made less than 24 hours will result in a cancellation fee. The amount of the fee will be equal to 50% of the reserved services. If you are more than 15 minutes late for your service, we may not be able to accommodate you. In this case, the same cancellation fee will apply.
We require a credit card to hold your appointment. Cancellation fees will be charged to your card on file.
